Upcoming changes to WorkCover WA Approved Forms
When the Workers Compensation and Injury Management Act 2023 (the Act) came into operation on 1 July 2024, many forms and processes changed in key areas for WorkCover WA and stakeholders.
As part of a review of administrative forms and processes, WorkCover WA sought feedback from stakeholders on a number of proposed changes to key WorkCover WA forms that came into operation on 1 July 2024.
After an extensive review and consultation process, WorkCover WA has finalised changes to the following approved forms:
- SF1 – Settlement Agreement
- SF3 – Permanent Impairment Notice
- CN1 – Intention to Reduce or Discontinue Income Compensation – Consent
- CN2 – Intention to Reduce or Discontinue Compensation – Return to Work
- CN6 – Custody or Imprisonment Notice
- APIA3 – Permanent Impairment Assessment Requirement to Attend
- APIA4 – Permanent Impairment Assessment – Provision of Information
- APIA5 – Permanent Impairment Assessment Consent to Provision of Information
- CF2 – NIHL Compensation Claim Form
- NIHL1 – NIHL Audiological Test Report
- NIHL3 – Worker Noise Exposure and Employment History
The new approved forms will come into operation on 1 July 2025 and can be found under the ‘WorkCover WA Approved Forms and Notices’ tab via this link.
Current versions of the above approved forms must continue to be used for now but should no longer be used after 30 June 2025.
It is important all stakeholders begin to use the new versions of the approved forms from 1 July 2025 to ensure the continuing smooth operation of the Act.
