Renewal applications
Workplace Rehabilitation Providers (WRPs) with approvals due to expire on 30 June 2016 are required to submit a renewal application to WorkCover WA by 31 December 2015.
Renewal applications can be submitted electronically or via hard copy.
If your originating jurisdiction is not Western Australia your application will be processed following notification of approval from that jurisdiction.
Revised renewal application form
WRPs should note the renewal application form has been revised to provide a more streamlined process. The renewal application and Part C – WorkCover WA Additional Renewal Requirements are available on this website.

The Heads of Workers’ Compensation Authorities have also revised the Guide: Nationally Consistent Approval Framework for Workplace Rehabilitation Provider. The guide should be reviewed in conjunction with your renewal application. The latest version is available through WorkCover WA’s website.
