Changes to approved forms from 1 July 2025
A range of new approved forms, updated after WorkCover WA conducted an extensive review and consultation process, come into operation 1 July 2025. The new approved forms can be accessed from the Legislative Framework: Approved Instruments, Forms and Notices page.
The following approved forms have been updated:
- SF1 – Settlement Agreement
- SF3 – Permanent Impairment Notice
- CN1 – Intention to Reduce or Discontinue Income Compensation – Consent
- CN2 – Intention to Reduce or Discontinue Compensation – Return to Work
- CN6 – Custody or Imprisonment Notice
- APIA3 – Permanent Impairment Assessment Requirement to Attend
- APIA4 – Permanent Impairment Assessment – Provision of Information
- APIA 5 – Permanent Impairment Assessment Consent to Provision of Information
- CF2 – NIHL Compensation Claim Form
- NIHL1 – NIHL Audiological Test Report
- NIHL3 – Worker Noise Exposure and Employment History
To ensure the continuing smooth operation of the Act, the updated approved forms on the WorkCover WA website should be used from 1 July 2025. Previous versions of the approved forms should no longer be used.
