New forms for Conciliation and Arbitration
On 2 December 2013 all existing WorkCover WA forms for the Workers’ Compensation Conciliation Service and Workers’ Compensation Arbitration Service will be replaced.
All stakeholders are expected to use and progress the new forms from this date forward. To facilitate the transition the Workers’ Compensation Conciliation Service and Workers’ Compensation Arbitration Service will continue to accept the old forms until 31 January 2014.
Due to content and layout changes, all legal representatives and registered agents participating in the workers’ compensation scheme in Western Australia should re-familiarise themselves with the forms used for Conciliation and Arbitration.
The new forms are accompanied by comprehensive completion instructions and have been designed to be more user-friendly, including lighter background colours for faxing.
The forms are only available in PDF format. For users wishing to fill the forms in electronically, an electronically fill-able PDF has superseded the Microsoft Word version.
