Updated Workers’ Compensation Claim Form

WorkCover WA has updated the Workers' Compensation Claim Form to reflect recent changes to the Workers' Compensation and Injury Management Act 1981.

The updated claim form needs to be completed by any worker seeking compensation for an injury sustained in the course of their employment.
