Revised workers' compensation claim form now available

The revised workers’ compensation claim form takes effect from 1 October 2010 and is now available on the WorkCover WA website.

Guidance material has been developed to help injured workers complete the form. This material explains what information needs to be included in the various sections of the claim form.

What has changed on the form?

Improvements to the form include:

  • detailed information about the claims process and entitlements
  • colour-coding and checklists which make the form easier to complete
  • date fields that will more accurately track delays in form lodgement, and
  • interactive fields which make it easier to complete online and then download.

Where can you get the form?

You can get your copy of the new claim form from:

  • The WorkCover WA website: This version can be completed online, saved and printed.
  • WorkCover WA’s Advisory Services: Injured workers can request a claim form by phoning 1300 794 744.
  • Your insurer: Employers should contact their insurer to order copies of the claim form.

If you are injured at work, you can request a claim form from your employer.

A two-month transitional period has now commenced, during which the old form will be phased out.
