Medical Certificates now available in word format

Content:

WorkCover WA is pleased to announce that based on feedback from medical practitioners, the first, progress and final medical certificates are now available in two formats on the WorkCover WA website:

1. Standard PDF - to print and complete by hand, and
2. Microsoft Word - to complete and save electronically, and to print out

The Word version of the medical certificates look different to the PDF versions as they have been rebranded, but they contain the same information.

Medical practitioners can:

  • complete the certificates on their computer and save them in their own file, and
  • print the completed version, sign and submit it the injured worker

Click on this link to access the medical certificates on the website: http://www.workcover.wa.gov.au/Publications+And+Forms/Forms/Medical+Forms.htm
